If your locations and availabilities have been setup, the app has been enabled, and the app is still not showing it's likely due to theme compatibility issues. The app was tested to work out of the box with all of Shopify's default free themes but if you have a custom theme the page structure may differ enough that the app widget will have problems displaying.
